Pail of water + kiddies sandbox = DIY quicksand.
One foot in the loo = sucked down in the bog.
Crucified on a clothes hanger (very un-PC).
Falling off laddered tights.
Freeze to death with an ice cube on the head
For the trekki: sucked into a Dyson vortex.

Those are brilliant, NMott. Are you going to post a link in Comedy Bill? I'm sure people will offer loads of ideas. LOve the bird flu one - I really related to that one - I keep skirting round ducks and giving swans a funny look.
